  Supplemental Patient Information Form

  Patient Instructions for Semen Collection

Be sure to have picture identification for confirmation by an Andrologist. This is vital to verification of identity for your specimen and is for your protection.

Payment in full, of most services offered by the Andrology Laboratory, is required at the time the specimen is received. Our office will be glad to submit a courtesy claim to your insurance for reimbursement to you.

Specimens
If you are bringing your specimen with you, be sure not to collect more than 1 hour before your appointment time in order to ensure optimal results. An Andrologist will review your collection form and very that your specimen is acceptable for processing.

If you are collecting your specimen at Virginia IVF and Andrology Center, you will be given the use of a comfortable, private room with DVD and print material available. Your partner may accompany you during the collection process if you prefer.

Test Results
If your appointment is for diagnostic semen testing, results of your test will be faxed to the ordering physician within 5 business days. Please follow-up with your physician for interpretation of the results. If you are not testing under physician’s order, you will receive a testing summary from us which should be taken to a physician for further interpretation and guidance.

Specimens for IUI appointments
If your appointment is for a semen wash, or sperm isolation, for intrauterine insemination, you may have your completed specimen delivered to your physician’s office by courier for an additional charge. If you or your partner choose to pick up the specimen, at the time of pick up you must be able to give the Andrologist the verification information given to the patient at the time the specimen was received and picture identification.
